TY - CHAP AB - The contribution deals with general hospitals, their physicians and the advanced stages of chronic diseases. It focusses on patterns of professional action identified when the oldest patients are being prepared for discharge from hospital. How do the physicians manage their function as gatekeepers, how do they interpret their own scope for discretion in the face of institutional regulations and system requirements to provide the oldest old with access to optimal follow-up care? How do they asess these patients, and their need for care? Among the striving forces governing the professional action the profession-specific intentions are all too often overlaid by orientations dictated by the health system and its structure. The result is the progressing loss of professional elements of the physician's action and - on the side of the patient - drastical reduction of potency for coping.
